اختلالات کروموزومی در مبتلایان به نارسايی زودرس تخمدان
Chromosomal abnormalities in women with Premature Ovarian Failure
زمینه و هدف: نارسايي زودرس تخمدان (POF) يا يائسگي زودرس، يكي از مشكلات استرسزا در خانمهاي جوانتر از 40 سال ميباشد كه مشكلات عديده روحي، جسمي و عوارض زودرس و ديررس را به دنبال دارد. عدم تكامل سلولهاي جنسي منجر
به نارسايي تخمدان و کاهش تعداد اين سلولها بیشتر به نارسایی نسبی تخمدان منجر میشود که نتیجه آن آمنوره ثانویه است. تاكنون مطالعهاي در مورد عوامل كروموزومي دخيل در نارسايي زودرس تخمدان در كشور انجام نشده است. این مطالعه برای بررسی اختلالات کروموزومی در مبتلایان به نارسایی زودرس تخمدان مراجعه كننده به بخش زنان و زایمان بیمارستان طالقانی انجام گرفت.
روش بررسی: 34 بیمار که بهدلیل قطع قاعدگی قبل از 40 سالگی و ناباروری متعاقب آن، از بهار سال 1384 الی تابستان 1385
به بخش زنان بیمارستان طالقانی دانشگاه علوم پزشكي شهيد بهشتي مراجعه کرده بودند مورد معاینه فیزیکی و سونوگرافی قرارگرفتند و تستهای هورمونی (PRL، LH، FSH) براي ايشان انجام شد. سطح FSH سرمی همه این بیماران مساوی یا بیش از IU/lit 40 بود. پس از آن مبتلایان برای مشاوره ژنتیک و آزمایش سیتوژنتیک (کاریوتایپ) به متخصص ژنتیک معرفی شدند.
نتايج: 8/81% افراد مورد بررسي كمتر از چهل سال و 2/18% چهل سال يا بيشتر سن داشتند. 6/17% این بیماران ( 6 نفر) با میانگین سنی 6/71/32 کروموزومهای X مشخصاً غیرطبیعی داشتند؛ بهطوريكه کاریوتایپهای موزائیک 45, X/46, XX یا 47,XXY (کلاین فلتر)، موزائیک 46, XX/47, XXX را نشان دادند.
نتیجهگیری: با وجود عدم مشاهده اختلاف معنیدار بین POF و آنومالی کروموزومی، اختلالات کروموزومی مشاهده شده در این مطالعه و در تعداد اندک از بیماران تحت بررسی که بهدلیل نارسایی زودرس تخمدان به یائسگی زودرس مبتلا شده بودند و در جستجوی راهی براي درمان یا روشی که به کمک آن بتوانند صاحب فرزند شوند و مادر بودن را تجربه کنند، مطالعه کروموزومی به عنوان یکی از قدمهای اولیه برای بیمار در جهت آگاهی از علت آمنوره ثانویه او پیشنهاد میشود. اطلاعات بهدست آمده از آزمونهای سیتوژنتیک مثل آنالیز کروموزومی برای مدیریت بیمار، مشاوره ژنتیک و برنامههای آینده فرد بیمار حائز اهمیت است.
Introduction: Premature ovarian failure (POF) or premature menopause is one of the stressful problems in women younger than 40, which may cause numerous early or late psychological and physical complications. Failure of germ cell development is associated with complete ovarian failure, while their decreased number is more likely associated with partial ovarian failure, which leads to secondary amenorrhea. Literature review shows almost no study has been done on chromosomal factors involved in POF in Iran. The purpose of this study was to study chromo-somal abnormalities in women with premature ovarian failure referred to the Fertility Health Research Center at Taleghani Hospital.
Materials & Methods: Thirty-four patients experiencing menopause before the age of 40 and resultant infertility, attended the gynecology and obstetrics ward of Taleghani Hospital of Shahid Beheshti University of Medical Sciences, from the spring of 2005 to the summer of 2006, and underwent physical exam, sonography and hormonal tests (FSH, LH and PRL). The FSH level of the cases was ≥40 IU/lit. Then, the patients were referred to a genetic counselor for undergoing cytogenetic tests and counseling.
Results: Most of the cases (81.8%) were younger than 40 years of age and 18.2% were 40 or older. Definite abnormal X chromosomes − mosaic karyotypes 45, X/46, XX or 47, XXY (Klinefelter’s syndrome) and mosaic 46, XX or 47, XXX − were diagnosed in 17.6% of the patients (6 cases) with a mean age of 32.1±7.6.
Conclusion: Although, there were no significant correlations between POF and chromosomal abnormalities, the results from the chromosomal analysis in this study and in a limited number of cases who had been affected by early menopause due to premature ovarian failure, it is suggested that chromosomal studies be considered as one of the first steps to understand the reasons for amenorrhea. The information obtained from cytogenic tests such as chromosomal analysis could be useful for patient management, genetic counseling, and future family planning.
