تاثیر حاملگی طبیعی بر میزان پلاسمایی لیپوپروتئین- آ
Effect of normal pregnancy on plasma levels of lipoprotein (a)
مقدمه: لیپوپروتئین- آ، یک ذره غنی از کلسترول در پلاسمای انسان میباشد، که بهعنوان یک عامل خطرساز مستقل برای آترواسکلروز شناخته شده است. علاوه بر بیماریهایی نظیر دیابت قندی و نارسایی کلیه در مواردي چون حاملگی طبیعی و پرخطر نیز ممکن است، غلظت پلاسمایی این لیپوپروتئین دچار تغيير گردد. از آنجا که غلظت پلاسمایی Lp(a) و تاثیر احتمالی حاملگی بر آن وابسته به نژاد می باشد، هدف از مطالعه حاضر بررسی اثر حاملگی بر غلظت پلاسمایی این لیپوپروتئین در گروهی از زنان باردار مقیم شهر یزد بود.
مواد و روشها: گروه مورد مطالعه شامل 94 زن باردار(میانگین سنی 33/46/24 سال) و 51 زن غیرباردار سالم
(میانگین سنی 73/62/26 سال) بود. نمونه خون در اول صبح و در حالت ناشتا تهیه شد و سرم حاصل در دماي C70- تا زمان سنجش Lp(a) ذخيره گرديد. پس از تعیین مقدار Lp(a)، نتایج حاصل مورد آنالیز آماری قرار گرفت. تستهای آماری U-test و تست Wilcoxon برای مقایسه Lp(a) در دو گروه(باردار و شاهد)، t-test برای مقایسه لیپیدها در دو گروه و تست Wallis Kruskal- برای مقایسه متغیرها در چهار گروه (سه ماهه اول، سه ماهه دوم، سه ماهه سوم و گروه شاهد) و آزمون همبستگی پیرسون برای بررسی ارتباط Lp(a) با سایر متغیرها مورد استفاده قرار گرفت. 05/0P< از نظر آماري معنيدار در نظر گرفته شد.
نتایج: ميانگين و انحراف معيار سطح پلاسمایی Lp(a) در گروه باردار (mg/dl 5/2225) بهطور معنیداری از گروه شاهد
(mg/dl 5/1318) بالاتر بود(01/0=P). در افراد مورد مطالعه سطح پلاسمائي Lp(a) همبستگی معنیداری با سن، میزان کلسترول و میزان تریگلیسرید نداشت. در 50 نفر از زنان باردار، غلظت پلاسمایی Lp(a) در نیمه دوم حاملگی (mg/dl 4/2231) بهطور معنیداری از نیمه اول (mg/dl 1620) بالاتر بود(001/0=P). مقایسه Lp(a) در گروههای سه ماهه و شاهد، افزایش معنی داری در گروه سه ماهه سوم نسبت به سایر گروهها نشان داده شد.
نتیجهگیری: نتایج این مطالعه نشان می دهد که مشابه با اغلب ساير جوامع مطالعه شده، غلظت Lp(a) پلاسمایی در افراد باردار بالاتر از افراد غيرباردار سالم بود. غلظت Lp(a) در هنگام حاملگی به تدریج افزایش مییابد و این افزایش در نیمه دوم حاملگي شدیدتر است. با توجه به مشکلاتی که افزايش Lp(a) پلاسمایی ممکن است در جریان خون جفت ایجاد نماید، پیشنهاد میگردد مطالعات بیشتری بهویژه در مورد رابطه این لیپوپروتئین با بروز پرهاکلامپسي، تولد نوزاد کم وزن و سابقه سقط مکرر صورت گیرد.
Introduction: Lipoprotein (a) [Lp(a)], a cholesterol- rich particle in human plasma is an indepe-ndent risk factor for atherosclerosis. In addition to diabetes mellitus and renal failure, normal and complicated pregnancy may affect the plasma levels of this lipoprotein. Since the effect of pregnancy and its complication on plasma level of Lp(a) is race dependent, the aim of present study was to evaluate the effect of pregnancy on plasma Lp(a) level in a group of normal preg-nant women living in Yazd City.
Materials and Methods: The study groups included 94 normal pregnant women (with a mean age 24.6 ± 4.33 years) and 51 non-pregnant women (mean age 26.2±6.73 years). Fasting blood samples were collected in the morning and sera were stored at –70 °C until Lp(a) was assayed. Statistical analysis included, U-test and Wilcoxon-test for comparison of Lp(a), t-test for compa-rison of lipids, Kruskal-Wallis-test for comparison of variables in four groups (first, second, third, trimesters and control) and Pearson correlation test, for correlation of Lp(a) to other variables.
Results: plasma Lp(a) levels in study group did not correlate significantly with age and other lipids, and in the pregnant group, (Mean±SD, 25±22.5 mg/dl) were significantly (p=0.01) higher than controls (18±13.5 mg/dl). Lp(a) plasma levels did not show any significant correlation with age, cholesterol and triglyceride levels. Plasma Lp(a) levels in the second half of pregnancy (31±22.4 mg/dl) was higher than the levels in the first half (20±16 mg/dl). Comparison of plasma Lp(a) in the third trimester and control showed a significant elevation in the third trimester.
Conclusion: The results of the present study indicate that like most other reports, plasma Lp(a) levels increased during normal pregnancy in our study population, and the elevation is more pronounced in the second half of pregnancy. Since very high levels of plasma Lp(a), may affect placental circulation, more studies are needed to clarify the possible role of this lipoprotein in complicated pregnancies such as pre-eclampsia, history of low birth weight and repeated fetal loss.
