مقايسه عوارض هيستركتومي ابدومينال در برابر هيستركتومي واژينال
Comparison of complications in abdominal hysterectomy versus vaginal hysterectomy
مقدمه: هيستركتومي يكي از رايجترين عملهاي جراحي در تمام دنياست. روش هيستركتومي خود به دو دسته اصلي شامل ابدومينال و واژينال تقسيم ميشود كه هر كدام از اين روشها انديكاسيون مخصوص خود را دارند؛ ولي گاهي ميتوانند انديكاسيون مشترك نيز داشته باشند. مهمترين انديكاسيونهاي هيستركتومي ابدومينال عبارتند از: فيبروم رحمي، AUB، PID مقاوم به درمان، اندومتريوز، بدخيميهاي رحم وتخمدان، آدنوميوز همراه با خونريزي واژينال، درد لگني مقاوم به درمان و CIN سرويكس. مهمترين انديكاسيون هيستركتومي واژينال نيز پرولاپس رحم در بيماران بدون سابقه چسبندگي لگن ميباشد. با توجه به اهميت و شيوع هيستروكتومي و مشكلات پس از آن تصميم گرفتيم تا مقايسهاي بين دو روش هيستروكتومي از نظر ميزان عوارض آن داشته باشيم تا بتوانيم در انتخاب روش هيستركتومي با دقت بيشتري عمل كنيم. اميد است كه اين مطالعه راهي جهت انتخاب روش صحيحتر هيستركتومي باشد.
مواد و روشها: اين مطالعه، يك مطالعة تحليلي از نوع مورد- شاهد (گذشته نگر) بر اساس اطلاعات موجود در پرونده بيماران ميباشد. مطالعه بر روي 339 بيمار هيستركتومي شده، 57 مورد (8/16%) به صورت واژينال و282 مورد (2/83% ) بهصورت ابدومينال، در طي سالهاي 81-80 در بخش زنان بيمارستان شهيد يحيي نژاد بابل انجام گرفت. در اين تحقيق متغيرهايي مانند سن بيماران، تعداد روزهاي بستري، ميزان افت هموگلوبين و ديگر عوارض زودرس در دو گروه هيستركتومي مورد مقايسه قرار گرفت. دادههاي حاصل بهوسيله نرم افزار SPSS وآزمونهاي آماري 2 ،t-test و Fiher’s exact و مدل لجستيك چندگانه و محاسبه نسبت خطرات تطبيق داده شده آناليز گرديد.
نتايج: متوسط سن بيماران در هيستركتومي واژينال 12 5/58 سال و ابدومينال 9/769/44 سال بود. ميزان كلي عوارض در هيستركتومي ابدومينال(7/23%) بيشتر از واژينال (3/5%) بود (01/0=P). تب مهمترين عارضه بعد از عمل بود كه در هيستركتومي ابدومينال (4/18%) بيشتر از واژينال (5/3%) مشاهده گرديد. از طرفي ميزان درصد افت هماتوكريت در هيستركتومي ابدومينال (05/239/2%) كمتر از نوع واژينال (32/176/3%) بود(001/0p<).
نتيجهگيري: با توجه به عوارض كمتر هيستركتومي واژينال در مطالعه حاضر و مطالعات مشابه انجام شده، در صورت عدم كنترانديكاسيون و امكان جايگزيني اين روش به جاي هيستركتومي ابدومينال استفاده، از آن در خانمهاي كانديد انجام هيستركتومي توصيه ميشود.
Introduction: Hysterectomy is one of the most common operations in the world. It is divided into two main abdominal and vaginal categories. Each type has usually specific indications but sometimes they have common indications. The main causes of abdominal hysterectomy are uterine fibroma, AUB, resistant PID, endometriosis, gynecologic malignancies, adenomyosis with vaginal bleeding, pelvic pain and cervical CIN and the most important indication for vaginal hysterectomy is uterine prolaps. Considering the importance and prevalence of hysterectomy and its side effects, we decided to compare the complications of vaginal versus abdominal hysteric-tomy to try to define more solid criteria for selecting hysterectomy method for the candidate patients.
Materials and Methods: This is a case-control (retrospective) analytic study.Data was collected from March 2001 until March 2003 of patient who had hysterectomies in GYN Department of Yahyanejad hospital in Babol. Of the 339 women who had undergone hysterectomy, 57 (16.8%) had received vaginal and 282 (83.2%) abdominal hysterectomy. The Variables Considered included age, days of hospitalization, decrease in hemoglobin concentration and other early side effects of hysterectomy. The data was analyzed by SPSS software Program using 2, t- test and fisher's exact test and multiple logistic model.
Results: The mean age of the Patients who had undergone hysterectomy was 58.512 years for Vaginal and 44.697.9 years for abdominal hysterectomy. The overall Complication rates were 23.7% and 5.3% (p=0.01) for abdominal and Vaginal hysterectomies, respectively. Fever was the main complication after surgery with a frequency of 18.4% and 3.5% for abdominal and vaginal hysterectomies, respectively. The decrease in hematocrit was lower in abdominal (2.392.05%) than in vaginal (3.761.32%) hysterectomy (p<0.001).
Conclusion: The results indicate that the overall complications in vaginal hysterectomy are less than abdominal which may suggest that vaginal hysterectomy may be an appropriate alternative for abdominal hysterectomy.
